About the course:

This course aims to identify patients at risk of Frailty, and to consider the best course of management.

What will I learn?

• To understand what frailty is and what it looks like
• To consider the effect of frailty on patients in the hospice, hospital, and community settings
• To be aware of the assessment process of frailty, including the electronic Frailty index and Rockwood clinical frailty score 
• To consider case studies and how we can best support patients with frailty.

Who should attend:

This course is aimed at clinical professionals involved with patients over the age of 18, who may have received a diagnosis of frailty, or may require an assessment of frailty.
